Payment is due at the time of treatment unless other arrangements have been made. We accept cash, checks, and all major credit cards. For your benefit, please bring your completed insurance forms with you at the time of your appointment.
SelfPay
We require that the services provided are paid in full by the time they are completed. If several visits are required, then 50% is due at the start of your procedure and the remainder is due by the completion of the procedure. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.
Insurance
Procedures and treatments for venous insufficiency that are medically necessary are covered by most health insurance plans. At The Vein Center, we accept Blue Cross Blue Shield, United Healthcare, Aetna, Cigna, Humana Medicare, and Medicare replacement plans.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.
